Erosion & Sedimentation Training On-line Registration Form
Please complete a registration form for each person attending.
A notification from our office will be sent to you within two business days to confirm your registration and seating in a training session. The confirmation letter and directions are sent by email, using the e-mail address on the online registration form. If the e-mail is returned, they will be faxed and/or mailed. On-line registration forms are to be used only when payment is going to be made using our secure on-line payment service (PayPal.com).
Registrants do not have to have their own PayPal account to make a credit card payment.
NOTES:
This form is only for online payments. Once you have clicked on the 'Submit Form' button you will be taken to a web page which will introduce you to our secure payment process through PayPal.com. For other payment arrangements use the mail-in registration form. Registration will not be considered until payment is received.
Training manual will be provided at training site. For Exam Only sessions, manual are returned after exam.
* Email Address: It is important that we have the address of the person attending so that we may communicate directly with that person on course details, if the need arises.
CANCELLATION POLICY:
All cancellations must be made 2 weeks prior to the class date. No refunds will be given to those who register and fail to attend. You may substitute someone in your place.
